Health Homes (Medicaid) Care Management

Health Homes care management services are available to Medicaid beneficiaries with qualifying chronic illnesses. Adults who have HIV/AIDS, diabetes, hypertension, mental health diagnoses, asthma, Hepatitis C, and a variety of other chronic conditions may be eligible for these services.

Health Homes Supported Housing

This program utilizes grant funds, realized by Medicaid Redesign Savings, to provide supportive housing services. Medicaid beneficiaries who are unstably housed and have utilized emergency or inpatient medical services multiple times in the last year may be eligible for short-term rental assistance and case management services.

HIV/AIDS Support Services

Have you recently found out you were living with HIV/AIDS? Or perhaps you are trying to learn how to be more independent in managing your medications, appointments, or other medical conditions? Either way, STAP has services that are tailored to where you are in both life and health.

Ryan White Case Management

Ryan White case management services are available to Persons Living with HIV/AIDS (PLWHA) who do not have Medicaid. This program provides comprehensive care coordination and navigation/referral services. Staff members will work with you in order to help ensure access to primary and HIV medical care, support services in the community, and benefits/entitlements for financial security.

Treatment Adherence Services

Treatment Adherence services are available to all PLWHA who are interested in additional education about HIV. The staff members in this initiative provide individualized education about a variety of HIV and related topics. By learning about medication adherence, resistance, or concurrent medical conditions, you will be better able to understand ways to improve your health and manage your viral load.

Housing Opportunities for People With HIV/AIDS (HOPWA) & Supportive Housing Services for Persons Living with HIV/AIDS

STAP operates two housing programs for PLWHA in the Southern Tier. Both programs offer a rental subsidy, as well as single-event financial assistance, to qualifying clients with limited incomes. The short-term subsidy program also provides housing case management services, to help ensure household financial stability, access to HIV and primary medical care, and referrals to community support services.

The Broome County Re-entry Program

The Broome County Re-entry Program (BCRP) assists formerly incarcerated men and women returning to Broome County to overcome barriers in order to transition back into the community. The Re-entry Program provides direct and referral services to meet the identified needs of each participant. BCRP provides opportunities for community education and collaboration in order to reduce barriers and recidivism.
